Cancellation Policy
Cancellation policy applies to all patients irrespective of booking source or appointment type. Same day booking cannot be cancelled or rescheduled for any reason whatsoever including act of god or force majeure.
Patients can do one free rescheduling within 24hours of booking (Does not apply to same day bookings). Appointment booking cannot be cancelled for any reason whatsoever including act of god or force majeure. Cancellation fee applies as follows.
1) Cancellation of a scheduled appointment will incur a fee of $199. Cancellations of procedure/therapy appointments will incur a fee of $499.
2) Missed or no-show scheduled appointments with or without notification incurs a fee of $199 for standard appointments and $499 for procedure/therapy visits.
3) Patient to reimburse Golden State Family Medicine for any booking charges incurred by Golden State Family Medicine due to use of 3rd party appointment booking websites by the patient.
4) All cancellation or no-show fees must be paid before a new appointment can be booked.
5) Any unpaid balance will be reported to credit bureaus and turned over to collection agencies.
